Disaster IQ Score you will need to change the email address in the
"From email address" field to your own email address when you submit
your completed form.
NOTE: If your email program does not function after you click on the Submit button your survey has not been submitted!!! The
problem is typically setting up the default email program through the
control panel of your computer. Go to your Control Panel, click on Add
or Remove Programs, click on Set Program Access and Defaults, expand
the Custom Configuration Section, choose your default email program AND make sure the Enable Access to this Program box is checked.
